Memory Care at Chartwell Guildwood 

Chartwell Guildwood’s Memory Living Neighbourhood is a supportive and independent environment for seniors living with dementia, located on a secure floor of our brand-new retirement residence in beautiful Guildwood Village. We are not only a safe and inviting home for seniors living with cognitive impairment, but also a resource for families who are looking for support, education and personal counselling services during and after a loved one’s transition to memory care.

    Our unique approach to memory care 

    At Chartwell Guildwood, our unique, person-centred approach to memory care empowers our trained and caring staff to establish strong social connections with residents in order to foster trust and familiarity. We do this by learning about each person’s life history and partnering with families—all in an effort to better understand your loved one’s story and form meaningful relationships that helps us better communicate and ensure your loved one experiences a good day, every day.

    Memory care support for families of residents 

    We understand that caring for a loved one living with dementia is difficult, although rewarding. That’s why to better support the families of our residents, we have a dedicated Dementia Counsellor on-site to provide orientation, facilitate family conferences, and deliver counseling services if desired. You and your family members are also be encouraged to drop by any time to speak with our Memory Living Manager.

    Chartwell Guildwood’s Memory Living Neighbourhood includes the following specialized programs and services:   - Small resident-to-staff ratios   - Secure and interactive environment with 24-hour supervision, including access to outdoor space   - Specialized life-enrichment programming, dining with staff, and group outings   - Flexible routines that balance structure with spontaneity   - Person-centred care plans reflective of residents’ unmet needs, abilities and interests   - Dedicated Memory Living Manager and Dementia Counsellor to support families
