Chartwell’s Care Assist Program is a seamless range of health and wellness or care services that are available in a retirement residence; services which adapt to your uniquely changing needs over time. The major benefit? Older adults maintain their independence for as long as possible, enjoying an active and social retirement lifestyle and can age in place with the help of supportive services too, helping to avoid a further move or the need for a long-term care setting.

Chartwell’s Care Assist Program is an offering of à la carte wellness services and all-inclusive care packages. Our program is designed to help you or a loved one achieve the comfort and quality of life you desire in your retirement residence. Delivered by the caring Chartwell staff you have come to know and trust; our highest priority is to help you feel your best each day for your complete peace of mind.
We know the people you place your trust in to provide your care are just as important—if not more so—than the services themselves. At Chartwell, our staff are compassionate, respectful, and friendly people who are dedicated to your health and happiness. We are here to get to know you personally so we can anticipate your needs, support your choice and independence, and employ the discretion and care you deserve.
Our well-trained care staff include dedicated and trained Personal Support Workers, Nurses, and other experienced staff that are truly committed to your wellness, with someone on-site 24/7. The call bell system in your suite also means that you have support available if and when you need it.
Additionally, Chartwell has partnered with several health and wellness providers, including lab and pharmacy services. This provides you with access to a pharmacist or other health care providers without the need to leave your retirement community.

Who Qualifies for Care Assist?
All Chartwell residents qualify for Care Assist, although there are circumstances where a resident's needs do not fit into our pre-existing packages. We want your living experience with us to be an exceptional one, where you always feel safe and supported. That’s why together with you and your family, we will collaborate to craft a customized plan of care that reflects your unique needs and goals:
Our Health & Wellness Manager will meet with you to conduct a review of your health needs, capabilities and interests, and understand your personal story.
Once we’ve established your plan of care, we’ll schedule health reviews to ensure we are adapting to your needs.
You have the flexibility to add or remove services at any time so you can comfortably age in place.
