Creating your personalized plan of care

We want your living experience with us to be an exceptional one, where you always feel safe and supported. That’s why together with you and your family, we will partner to craft a customized plan of care that reflects your unique needs and goals.

1. Our Health & Wellness Manager will meet with you to conduct a review of your health needs, capabilities and interests, learning your personal story.
2. Once we’ve established your plan of care, we’ll schedule health reviews to ensure we are adapting well to your needs.
3. You have the flexibility to add or remove services at any time so you can comfortably age in place.
